Got the bushing, and it works perfectly for holding the tool in the bore. The next problem to deal with is that it is now clear that the bore is not drilled square with the face. I think when I get time, I'll try to enlarge the bore a little so that I can use the shim tape to line things up. Another possibility may be to have the bore milled out to a larger diameter by a machine shop, this time square, and then get a slightly bigger bushing. Open to any other ideas.
